A recent study from 2010 looked to compare the success of spinal manipulation compared with microdiskectomy surgery in patients afflicted with sciatica stemming from lumbar disk herniation (LDH).

A total of 120 study subjects who’d been elected for research from primary care physicians to neurosurgical spine surgeons were individually evaluated for signs of unilateral lumbar radiculopathy sourced from LDH at either L3-4, L4-5, or L5-S1. Researchers realized that 40 straight consenting participants who met study inclusion criteria, which asked for patients to have failed three months or more of nonoperative management (such as analgesics, lifestyle modification, physiotherapy, massage therapy, and/or acupuncture treatment), were randomized to microdiskectomy surgery or expert chiropractic spinal manipulation. Patients were allowed to change therapy types if they so wished after three months.

Noticeable improvement was actualized in both treatment groups over time in all outcome measures; following a full year, follow-up intent-to-treat evaluation failed to show a discernible variance in outcome based on the initial therapy type administered. That being said, three patients did decide to switch from surgery to spinal manipulation.

In conclusion, a significant percentage of patients (60 percent) suffering from sciatica who hadn’t experienced relief using other medical intervention were found to benefit from spinal manipulation as well as if they had invasive surgery to correct their affliction. Patients who experience symptomatic LDH after feeling little to no relief with traditional medical management could experience discernible symptom relief following spinal manipulation.
